When a turn declines to do something

When a turn declines to actually carry out an action, whether because you answered no to a permission prompt or because plan mode recorded the action instead of running it, that decline also appears directly in the transcript.

It is never folded invisibly into the closing summary as if it simply had not come up. This means it stays clear afterward exactly which of the things tCode wanted to do actually happened and which did not, even reviewing the transcript well after the fact.

This transparency is part of what makes the closing summary trustworthy: it reflects what genuinely happened, not an idealized version of the turn. See Permissions and safety for how a decline is handled in the model's subsequent reasoning.

A decline recorded in plan mode, an action deferred rather than run, and a decline answered directly at a live permission prompt both surface the same way in the transcript: clearly marked as not having actually executed, distinct from anything that did.

Reviewing a transcript afterward specifically for what did not happen is a legitimate reason to reread one, not just for what did; both are equally part of an accurate record of the turn.
